Frailty affects a large proportion of senior Albertans. In efforts to operate more effectively and efficiently, healthcare systems in Alberta are adopting a “right provider, in the right place, at the right time” approach. For seniors’ care, this means delivering more health and social services in community settings. Leveraging Community-Based Senior-Serving (CBSS) organizations as key contributors to health promotion and prevention can prevent seniors from moving up the tiers of care and cost. However, this shift presumes a readiness amongst CBSS organizations. This project will facilitate and inform an effective shift in the delivery of some assessment and care provided to seniors in Alberta away from primary-care settings to community-based settings.
